- 摘要:目的 对比数字化塑形钛网与术中手工塑形钛网修补颅骨缺损塑形效果,评价数字化三维成型钛网技术在颅骨修补中的临床应用价值。方法 60例颅骨缺损患者随机分为数字化钛网修复组和术中手工塑形对照组,各30例,前者采用计算机辅助设计制造和快速自动成型技术,将钛网制成个性化颅骨修复体进行修补,后者采用术中手工塑形钛网修补。比较两组间手术时间长短、术中所需螺钉数目、术后随访患者满意程度以及术后并发症。结果 数字化塑形钛网组手术时间比手工组缩短约60min(P<0.05),钛钉用量明显少于手工塑形组(P<0.05),患者满意率(96.7%)优于手工塑形组(86.7%),术后并发症显著减少。结论 与传统术中手工塑形相比,数字化塑形钛网颅骨缺损修复术具有手术操作简单快捷、塑形的钛修复体高精准度恢复生理曲度外形、术后并发症少等多重优势,能够为患者提供完全个性化的治疗方案,具有广阔的应用前景。
- Abstract:
- ABSTRACT: Objective To assess the clinical application of digital titanium mesh shaping (DTMS) in skull neoplasty by comparing DTMS with manual shaping (MS). Methods Sixty patients undergoing cranioplasty were randomly divided into DTMS group and MS group, 30 cases in each. In DTMS group computer-aided design was made and fast automatic modeling technique was used to shape the titanium mesh into individual skull prosthetic replacement. Operation duration, the number of shaping screws needed in operation, patients’ satisfaction rate and post-operative complications were compared and analyzed. Results Compared with those in MS group, in DTMS group the average operation duration was significantly about 60 minutes shorter (P<0.05) and the titanium screws were fewer (P<0.05). The rate of patients’ satisfaction about the outcomes reached 96.7% in DTMS group, higher than that in MS group (86.7%). No other post-operative complications occurred in DTMS group except for transient subcutaneous effusion in one case. Conclusion Three-dimensional digital titanium mesh shaping prosthesis, with many advantages such as simple and convenient operation process, accurate and natural physiological curve and shape, as well as reducing complications and medical expenses, is superior to the traditional manual shaping and has a promising application prospect.
